About EOSS

The Edmonton Obesity Staging System (EOSS) classifies obesity into 5 stages (0-4) based on metabolic risk factors, physical symptoms, and psychological health. Unlike BMI alone, EOSS predicts mortality risk and guides treatment allocation. Stages 0-1 are not associated with increased mortality, while stages 2-4 require increasingly intensive intervention. The overall stage equals the highest domain score.

Domain Assessment

Metabolic

Stage 1Subclinical

Borderline HTN, impaired fasting glucose, mildly elevated lipids

Stage 2Established

HTN requiring treatment, Type 2 Diabetes, sleep apnea, dyslipidemia requiring treatment

Stage 3Significant

End-organ damage: MI, heart failure, diabetic complications (retinopathy, nephropathy, neuropathy)

Stage 4Severe

End-stage conditions: severe heart failure, end-stage renal disease, severe diabetic complications
